4. Does anyone know you cut? one knows that i used to...doesn't know that i still do
5. Does your parent(s)/ guardian(s) know? not yet
6. Do you ever burn or bruise yourself? yea
7. Do you think that you are depressed? yea i know i am
8. Do you think you have anxiety problems? haha def
9. Do you think you have Obsessive Compulsive Disorder (OCD)? a mild case of it
10. Do you have any other disorders? yes
11. Do you have an eating disorder? yes
12. Are you on meds? (if yes what?) not yet...avoiding them
13. Have you been hospitalized or been to the ER for self injury/ suicide attempt? no
14. Have you tried to commit suicide? (multiple times how many?) a few...i don't remember
15. How? overdoses
16. Does anyone know that you tried? one person thinks that i might have...but doesn't know for sure
17. Are you are have you ever been abused in any way? yea
18. Have you ever been sexually assaulted? thankfully no
19. How ( if you don't mind saying)?
20. Do you do drugs? (what, how much, how often)? no
21. Do you think of cutting as a good or bad thing?( keep you from trying to die, doing something worse?) it helps me, it's my way of relieving stress, and frankly keeps me alive and not doing other things
22. Do you wish you could stop? at times
23. Do you wish to stop in the future? yes
24. What are your feelings leading up to, during and after cutting? before anxious/stressed/upset, during relaxed and relieved, after relief and guilt and it all depends
25. When you cut does it hurt? How does it feel? no
26. Do you bandage your cuts? only if they won't stop bleeding quickly and i have to go to class or something
27. Where do you most normally cut? legs
28. Where do you cut? anywhere
29. Are you alone? yes
30. Do you have a weapon of choice, do you carry it with you? razors, i have some in my backpack, and saftey pins in my purse
31. Have you ever made a weapon out of something ordinary or something you found? yep
32. What have you used to injure yourself? anything i could find
33. How do you feel about your cuts/scars? they make me sad because they remind me of what i was going through, but they're somehow comforting also

35. What did you tell them? haha my cat's a b*tch
36. Do you ever show people your cuts/scars? (why or why not and who?) no - i don't want people to know
37. Do you ever design a cut (make it decorative or in a certain shape)? once...but that would be a harder scar to explain...cat's don't typically attack in patterns
38. Are your cuts lines or in rows, or designs? mostly lines, but some are random
39. Have you ever cut too deep? yea
40. Have you ever scared yourself while, before or after cutting? yea
41. Do you go to therapy? yea, i just started a few weeks ago and have gone twice
42. Do you have trust issues? trust? haha what a funny word - i don't believe in trust. i guess that would mean that i do have trust issues, yes?

44. Do you know people or hang out with people who cut? my friend used to but i got her to stop
45. Are you involved with any one right now? odd situation, he thinks so but i can't stand the thought of trying to trust him and i hate it when people touch me and i'm too gross looking to have a relationship
46. Do you have long relationships or one night stands, or two week relationships (1 month, 3 months, ect ect)? i prefer none, but long would be better
47. Does the person you are involved with know? Do they understand? no
48. Are you a virgin? ( when did you lose it) yea
49. Are you sexually active? no *see above*
50. Do you use protection? well oddly enough i didn't lose my virginity between questions 48 and 50, so i'd still say no since, well i'm still a virgin
